Individuals with active psoriatic arthritis who have failed to respond adequately to at least one prior Disease-Modifying Anti-Rheumatic Drug (DMARD) or anti-TNF therapy are eligible for coverage.

For individuals diagnosed with moderately to severely active Crohn's disease, Stelara is covered if they have failed or are intolerant to corticosteroids, immunomodulators, or TNF blockers.

Coverage is also provided for patients with moderately to severely active ulcerative colitis who have demonstrated a failure or intolerance to conventional therapy, including corticosteroids, immunomodulators, or have had an inadequate response to TNF blockers.

The patient must meet specific lab test requirements, ensuring that they do not have a significant infection and have ruled out potential pre-existing conditions that could interfere with the treatment.
