Criteria for Coverage of Orencia
<li>Patients must be 18 years or older, as Orencia is typically prescribed for adult use under Geisinger Health Plan coverage.</li>
<li>For pediatric patients, Orencia coverage is considered for those aged 2 years and older with a diagnosis of juvenile idiopathic arthritis (JIA), showing inadequate response to one or more disease-modifying antirheumatic drugs (DMARDs).</li>
<li>Individuals diagnosed with psoriatic arthritis may be covered if they have tried and had inadequate responses to at least one traditional DMARD or non-biologic therapy.</li>
<li>Certain documentation is required to validate the diagnosis, which may include clinical assessments, lab results, or imaging studies relevant to validate the patient profile and diagnosis.</li>
<li>Orencia must be prescribed by a healthcare provider specializing in rheumatology, immunology, or a related field.</li>
<li>There is a requirement that the prescribed dosage does not exceed the FDA-approved dosing recommendations for the particular condition being treated.</li>
<li>Certain biologic therapies or drugs should not be used in conjunction with Orencia, and patients must not be taking concurrent Tumor Necrosis Factor (TNF) inhibitors.</li>
<li>Reauthorization for continued treatment with Orencia requires documentation of therapeutic response and improvements, demonstrated by objective measures, following initial or prior approval phase.</li>
