Does Kaiser Permanente Cover Wegovy?

The verdict

Conditional on most plans

Kaiser Permanente's Wegovy coverage depends entirely on region. California base plans exclude GLP-1s for weight loss if BMI is below 40 (since January 2025). The Northwest region covers it with strict criteria including an unusual intra-molecule step-therapy requirement โ€” a failed trial of Ozempic before Wegovy is approved. The Mid-Atlantic requires documented 5% weight loss in the prior three months for continuation authorization. Kaiser's closed-network model means members cannot shop outside the system. Medicare covers the cardiovascular indication only.

Key facts

  • Kaiser California base plans exclude GLP-1 weight-loss drugs for new starts with BMI below 40 (since January 2025) โ€” a more restrictive threshold than Wegovy's FDA indication (BMI 30+), matching the Blue Shield of California restriction applied at the same time.

Kaiser Permanente California, 2025 formulary update; Blue Shield of California 2025 GLP-1 policy

  • Kaiser Northwest requires a failed adequate trial of at least 3 months of Ozempic (semaglutide for diabetes/CV) before authorizing Wegovy for weight-loss โ€” an unusual intra-molecule step-therapy requirement not found at most commercial plans.

Kaiser Permanente Northwest, prior authorization criteria for GLP-1 anti-obesity agents

  • California Medi-Cal dropped GLP-1 obesity coverage effective January 1, 2026, eliminating the weight-loss Wegovy pathway for Kaiser Medi-Cal members in California.

Plan types

Medicare vs. commercial coverage

Whether Kaiser Permanente covers Wegovy depends heavily on which kind of plan you hold. They follow completely different rules โ€” here's each one, side by side.

Medicare

Part D & Advantage plans

Conditional

Medicare Part D cannot cover Wegovy for weight loss by federal statute. For Kaiser Permanente Medicare members with established cardiovascular disease who are overweight or obese, the cardiovascular risk reduction indication is coverable under Part D with prior authorization. The $2,100 beneficiary OOP cap applies in 2026. The Medicare GLP-1 Bridge (launching July 1, 2026, through December 31, 2027) provides weight-loss access to Wegovy at a fixed $50 per month copay outside Part D โ€” this does not count toward the deductible, TrOOP, or the $2,100 cap. Bridge eligibility requires provider attestation: BMI 35 or higher; or BMI 30 or higher with heart failure, uncontrolled hypertension, or CKD; or BMI 27 or higher with prediabetes, prior MI, prior stroke, or symptomatic PAD. Members already receiving a Part D-covered GLP-1 are ineligible. Medicare beneficiaries cannot use the NovoCare savings card (federal anti-kickback rules). California Medi-Cal dropped GLP-1 obesity coverage effective January 1, 2026; non-weight-loss FDA indications remain covered.

Commercial / Employer

Group & marketplace plans

Conditional

Kaiser is a closed-network, regionally operated HMO โ€” members cannot go outside the system to shop for a plan with different Wegovy coverage. Coverage is region-dependent: California: Base Kaiser CA plans exclude GLP-1s for weight loss if BMI is below 40 (Class III obesity). This restriction took effect January 1, 2025 for new starts. Members with BMI 40 or higher may be covered with prior authorization. California Medi-Cal dropped GLP-1 obesity coverage effective January 1, 2026. Northwest (OR, WA): Kaiser Northwest covers Wegovy with strict criteria, including an unusual intra-molecule step-therapy requirement: members must document a failed adequate trial (at least 3 months) of semaglutide in the form of Ozempic before Wegovy is approved. This step, combined with BMI criteria and a documented lifestyle modification program, reflects Kaiser NW's conservative formulary approach. Mid-Atlantic (VA, MD, DC): Kaiser Mid-Atlantic requires prior authorization, including documented 5% weight loss in the prior 3 months for continuation authorization. Verify the specific criteria on the Kaiser Mid-Atlantic formulary. In all regions, the anti-obesity benefit is also employer/plan-dependent โ€” verify with Kaiser member services for your specific plan.

Prior authorization

The 6 boxes you must tick to get approved

Commercial plans that will consider Wegovy require prior authorization. Check off what applies to you to see how close you are โ€” this is a guide, not a guarantee of approval.

Requirements you meet0 / 6

Meet all 6 and you have a documented case for approval. Missing any one is the most common reason requests are denied.

What you'll pay

Your cost, three ways

What Wegovy actually costs you swings enormously depending on how it's covered. Here's the realistic range.

No coverage

Paying cash

$1,349 per month (Novo Nordisk published list price for a 28-day supply); this is the default for CA Kaiser members with BMI below 40

List price with no coverage โ€” before any manufacturer savings card.

PA approved

With coverage

Commercial cost-share where covered: plan-specific; varies by Kaiser region and plan tier. NovoCare savings card: not straightforwardly applicable within Kaiser's closed-network internal pharmacy model in the same way as retail-network plans; verify with Kaiser. Medicare CV indication: Part D cost-share depends on tier and benefit phase; $2,100 OOP cap 2026. Medicare Bridge (weight loss, from 7/1/2026): fixed $50 per month outside Part D.

Typical monthly copay once prior authorization is approved.

Lowest cost

Bridge program

NovoCare self-pay program: approximately $199 per month for starting doses (0.25/0.5 mg, first two fills through December 31, 2026), then approximately $349 per month for standard doses. Not available to Medicare or Medicaid beneficiaries. California Medi-Cal members are excluded from this GLP-1 benefit. From January 1, 2027, Novo Nordisk has announced a list-price reduction to approximately $675 per month; the IRA-negotiated Maximum Fair Price of $274 per 30-day supply also takes effect January 1, 2027.

Reduced copay through the assistance program, where eligible.

All figures are estimates. Actual cost-share varies by Kaiser region, plan tier, and pharmacy. Because Kaiser uses its own internal pharmacies, savings card mechanics may differ from retail networks. Confirm with Kaiser member services.

Do it yourself

Check YOUR exact coverage in 3 steps

General answers only get you so far. Here's how to confirm your own plan's decision in a few minutes.

  1. 1

Identify your Kaiser region and check regional formulary

Kaiser Permanente operates as separate regional entities โ€” California, Northwest, Mid-Atlantic, and others. The coverage answer for Wegovy differs by region. Visit your regional Kaiser member portal and search Wegovy (semaglutide 2.4 mg) on your specific plan's formulary to see the applicable tier and PA criteria.

  2. 2

Confirm your BMI and regional threshold with your Kaiser provider

California members: confirm whether your BMI meets the 40-or-higher threshold for weight-loss coverage. Northwest members: confirm whether you have completed (or can complete) the required prior Ozempic trial. Mid-Atlantic members: confirm you can document the 5% weight-loss continuation requirement. Your Kaiser provider will submit the prior authorization within the Kaiser integrated system.

  3. 3

Ask about the CV indication or Bridge eligibility if weight-loss criteria are not met

If your BMI is below 40 (California) or you cannot complete the Ozempic step (Northwest), ask your Kaiser provider whether the cardiovascular risk reduction indication applies. Medicare members should ask about GLP-1 Bridge eligibility (launching July 1, 2026, $50/month outside Part D).

If it's denied

Options if Kaiser Permanente won't cover Wegovy

A denial isn't the end of the road. These are the paths people commonly explore next โ€” discuss any of them with your prescriber.

Kaiser NW: complete prior Ozempic trial to unlock Wegovy

Kaiser Northwest members who have not yet tried Ozempic (semaglutide for diabetes/CV) should discuss with their Kaiser provider whether an Ozempic trial to satisfy the step-therapy requirement is an option, if they otherwise meet the BMI and lifestyle criteria for Wegovy.

Cardiovascular indication (Medicare Part D or commercial)

Wegovy is FDA-approved for cardiovascular risk reduction in adults with established CVD who are overweight or obese. This indication may be coverable under Medicare Part D or certain Kaiser commercial plans with prior authorization, distinct from the regional weight-loss criteria.

Medicare GLP-1 Bridge

Launching July 1, 2026, the Bridge provides eligible Medicare beneficiaries weight-loss access to Wegovy at $50 per month through December 31, 2027, outside Part D. Eligibility criteria apply; members already on a Part D-covered GLP-1 are ineligible.

Compounded semaglutide

Compounded semaglutide, available through licensed telehealth platforms, does not require insurance prior authorization. Starting from $49 per month. Compounded semaglutide is not the same medication as Wegovy and has not been approved by the FDA.

โ“Frequently Asked Questions

It depends on your region. California base plans exclude GLP-1s for weight loss if your BMI is below 40 (since January 2025). Kaiser Northwest covers it with strict criteria including a mandatory prior Ozempic trial of at least 3 months. Kaiser Mid-Atlantic requires documented 5% weight loss in the prior 3 months for continuation. Because Kaiser is a closed-network HMO, you cannot go outside the system to find a plan with different rules. Verify on your regional Kaiser formulary.

Starting January 1, 2025, Kaiser CA base plans limited new weight-loss GLP-1 starts to members with Class III obesity (BMI 40 or higher), a more restrictive threshold than Wegovy's FDA-approved indication (BMI 30 or higher, or 27 or higher with a comorbidity). This reflects a cost-containment decision by Kaiser CA โ€” and Blue Shield of California applied an identical BMI-40 restriction at the same time. Members with BMI 27โ€“39 who do not meet this threshold are excluded from weight-loss coverage under base California Kaiser plans.

Kaiser Northwest requires members to complete an adequate trial of semaglutide in its Ozempic formulation (at least 3 months) before Wegovy can be approved for weight-loss use. This is an intra-molecule step therapy โ€” using the diabetes form of the same molecule before the weight-loss form โ€” which is unusual; most plans do not require a diabetes GLP-1 trial before approving a weight-loss GLP-1. Members who have already used Ozempic may be able to document that trial; members who have not will need to complete it within the Kaiser NW system.

Both Kaiser CA and Blue Shield of California applied the BMI-40 restriction for new weight-loss GLP-1 starts effective January 1, 2025. The key difference is network model: Kaiser is a closed-network HMO where members receive care and pharmacy services entirely within the Kaiser system, while Blue Shield of California is an open-network plan where members can access a broader network of providers and pharmacies. Kaiser NW and Mid-Atlantic regions also have region-specific criteria that Blue Shield of CA does not apply.

No. California Medi-Cal dropped GLP-1 obesity coverage effective January 1, 2026, following other states in trimming GLP-1 obesity spending. Kaiser Medi-Cal members in California cannot access Wegovy for weight loss through the Medi-Cal benefit. The cardiovascular risk reduction indication remains covered for members with established CVD. Wegovy is not a diabetes drug and cannot be covered through a diabetes benefit.
