Does UnitedHealthcare Cover Wegovy?

The verdict

Conditional on most plans

UnitedHealthcare, through OptumRx, covers Wegovy on its standard formulary for cardiovascular risk reduction only. Weight-loss coverage requires an employer opt-in; where the employer opted in, a 4-month initial supply is authorized with continuation requiring documented 5% or greater weight loss. In February 2026, OptumRx removed the BMI threshold from its cardiovascular reauthorization criteria. Medicare does not cover Wegovy for weight loss by statute.

Key facts

  • OptumRx's standard formulary covers Wegovy for cardiovascular risk reduction only (since January 2025); weight-loss coverage requires an employer opt-in, making UHC a PBM-CV-standard plan distinct from CVS Caremark (which prefers Wegovy for weight loss) and Express Scripts (which excludes it on the base NPF).

OptumRx, 2026 formulary update and prior authorization criteria

  • In February 2026, OptumRx removed the BMI requirement from its cardiovascular reauthorization criteria for Wegovy, simplifying continued authorization for UHC members using the CV indication.

OptumRx, February 2026 prior authorization criteria update

  • Wegovy is FDA-approved for chronic weight management (BMI 30+, or 27+ with a weight-related comorbidity) and for cardiovascular risk reduction in adults with established CVD โ€” it is a weight-loss and CV drug, not a diabetes drug.

Plan types

Medicare vs. commercial coverage

Whether UnitedHealthcare covers Wegovy depends heavily on which kind of plan you hold. They follow completely different rules โ€” here's each one, side by side.

Medicare

Part D & Advantage plans

Conditional

Medicare Part D cannot cover Wegovy for weight loss by federal statute. For UHC Medicare Advantage members with established cardiovascular disease who are overweight or obese, the cardiovascular risk reduction indication is coverable under Part D with prior authorization โ€” OptumRx's 12-month authorization applies, and the February 2026 update removed the BMI requirement from CV reauthorization criteria. The $2,100 beneficiary OOP cap applies in 2026. The Medicare GLP-1 Bridge (launching July 1, 2026, through December 31, 2027) provides weight-loss access to Wegovy at a fixed $50 per month copay outside Part D โ€” this does not count toward the deductible, TrOOP, or the $2,100 cap. Bridge eligibility requires provider attestation: BMI 35 or higher; or BMI 30 or higher with heart failure, uncontrolled hypertension, or CKD; or BMI 27 or higher with prediabetes, prior MI, prior stroke, or symptomatic PAD. Members already on a Part D-covered GLP-1 are ineligible. Medicare beneficiaries cannot use the NovoCare savings card.

Commercial / Employer

Group & marketplace plans

Conditional

UnitedHealthcare uses OptumRx as its PBM. On the OptumRx standard formulary, Wegovy is covered for cardiovascular risk reduction only (since January 2025) โ€” weight-loss coverage is not available without an employer opt-in. Where the employer purchased the anti-obesity benefit, OptumRx grants a 4-month initial supply authorization; continuation requires documentation of at least 5% baseline weight loss. ACA/individual plans generally exclude Wegovy for weight loss. State Medicaid coverage through UHC varies โ€” only 13 state Medicaid programs cover GLP-1s for obesity as of January 2026; all states cover non-weight-loss FDA indications. If the employer did not opt in, weight-loss coverage is a benefit exclusion โ€” not appealable through a PA process. Verify coverage on the UHC member drug-lookup tool.

Prior authorization

The 5 boxes you must tick to get approved

Commercial plans that will consider Wegovy require prior authorization. Check off what applies to you to see how close you are โ€” this is a guide, not a guarantee of approval.

Meet all 5 and you have a documented case for approval. Missing any one is the most common reason requests are denied.

What you'll pay

Your cost, three ways

What Wegovy actually costs you swings enormously depending on how it's covered. Here's the realistic range.

No coverage

Paying cash

$1,349 per month (Novo Nordisk published list price for a 28-day supply)

List price with no coverage โ€” before any manufacturer savings card.

With coverage

Commercial cost-share where the employer opted in (weight loss) or CV standard: plan-specific; varies by tier. NovoCare Wegovy Savings Offer: commercially insured UHC members whose plan covers Wegovy may pay as little as $25 per month (max savings $100 per 28-day supply). Medicare CV indication: Part D cost-share depends on tier and benefit phase; $2,100 OOP cap 2026. Medicare Bridge (weight loss, from 7/1/2026): fixed $50 per month outside Part D.

Typical monthly copay once prior authorization is approved.

Lowest cost

Bridge program

NovoCare self-pay program: approximately $199 per month for starting doses (0.25/0.5 mg, first two fills through December 31, 2026), then approximately $349 per month for standard doses. Not available to Medicare or Medicaid beneficiaries. From January 1, 2027, Novo Nordisk has announced a list-price reduction to approximately $675 per month; the IRA-negotiated Maximum Fair Price of $274 per 30-day supply also takes effect January 1, 2027.

Reduced copay through the assistance program, where eligible.

Do it yourself

Check YOUR exact coverage in 3 steps

General answers only get you so far. Here's how to confirm your own plan's decision in a few minutes.

  1. 1

Run Wegovy through the UHC drug-lookup tool to confirm standard vs. opt-in coverage

Visit the UnitedHealthcare member portal and search for Wegovy (semaglutide 2.4 mg). The result will show whether your plan covers it for the CV indication only (standard OptumRx) or also for weight loss (employer opt-in). Confirm the tier and authorization requirements.

  2. 2

Ask HR whether the employer opted into weight-loss drug coverage

Because OptumRx standard formulary excludes weight-loss Wegovy, weight-loss coverage requires an employer opt-in. Ask your HR department directly whether anti-obesity drugs are included in your benefit design. This is the step that most competitor pages omit.

  3. 3

Ask your prescriber about the CV indication or Bridge eligibility

If you have established cardiovascular disease and are overweight or obese, the CV indication is available on the standard OptumRx formulary with a 12-month authorization (BMI requirement removed at reauthorization since February 2026). Medicare members should ask about GLP-1 Bridge eligibility (launching July 1, 2026, $50/month outside Part D).

If it's denied

Options if UnitedHealthcare won't cover Wegovy

A denial isn't the end of the road. These are the paths people commonly explore next โ€” discuss any of them with your prescriber.

CV indication via OptumRx standard formulary

Wegovy's cardiovascular risk reduction indication is covered on the OptumRx standard formulary for UHC commercial and Medicare members with established CVD who are overweight or obese. A 12-month authorization applies; the BMI requirement was removed from CV reauthorization criteria in February 2026.

Medicare GLP-1 Bridge

Launching July 1, 2026, the Bridge provides eligible Medicare beneficiaries weight-loss access to Wegovy at $50 per month through December 31, 2027, outside Part D. Eligibility criteria apply; members already on a Part D-covered GLP-1 are ineligible.

NovoCare self-pay program

For members without weight-loss coverage, Novo Nordisk's NovoCare program offers approximately $199 per month for starting doses (first two fills through December 31, 2026), then approximately $349 per month for standard injection doses. Not available to Medicare or Medicaid beneficiaries.

Compounded semaglutide

Compounded semaglutide, available through licensed telehealth platforms, does not require insurance prior authorization. Starting from $49 per month. Compounded semaglutide is not the same medication as Wegovy and has not been approved by the FDA.

โ“Frequently Asked Questions

Only if your employer opted into the anti-obesity benefit. OptumRx's standard formulary covers Wegovy for cardiovascular risk reduction only โ€” weight-loss coverage is not available on the base formulary. Where the employer opted in, OptumRx grants a 4-month initial supply; continued authorization requires at least 5% documented weight loss. ACA/individual plans generally exclude it. Medicare covers the CV indication only.

Where the UHC employer opted into anti-obesity coverage, OptumRx authorizes a 4-month initial supply of Wegovy for weight-loss use. This is an initial trial period. After 4 months, OptumRx requires documentation of at least 5% weight loss from baseline to authorize continued supply. Failure to demonstrate the 5% threshold at reauthorization can result in a coverage denial going forward, even if the drug is providing health benefits in other ways.

In February 2026, OptumRx removed the BMI requirement from its cardiovascular reauthorization criteria for Wegovy. This means that UHC commercial and Medicare members using Wegovy for the cardiovascular indication no longer need to meet a BMI threshold to continue authorization โ€” simplifying the renewal process. The initial CV authorization still requires documentation of established CVD and overweight or obese status.

Both UHC and Cigna have PBM standard formularies that exclude weight-loss GLP-1s, but the mechanics differ. OptumRx's standard formulary covers Wegovy for the CV indication (since January 2025) and requires an employer opt-in plus a 4-month initial supply for weight-loss coverage. Express Scripts' National Preferred Formulary excludes Wegovy injection and Wegovy tablets on the base formulary for 2026, with an Evernorth $200/month OOP cap available to participating employer plans. Both are PBM-driven, but OptumRx's CV standard coverage and 4-month initial supply structure are distinct from ESI's base exclusion plus cap model.

Commercially insured UHC members whose plan covers Wegovy (whether for CV or weight loss with employer opt-in) may use the NovoCare Wegovy Savings Offer (as little as $25 per month, max savings $100 per 28-day supply). The card is not available to Medicare or Medicaid beneficiaries. If your UHC plan covers only the CV indication and you are seeking weight-loss coverage not covered under your plan, the savings card applies only to covered uses at in-network pharmacies.
