The connection to rosemary is through the shared Lamiaceae family terpene chemistry: camphor and 1,8-cineole appear across rosemary, sage, lavender, and related herbs. A patient sensitized to rosemary via carnosol may show separate positive patch tests to sage terpenes through independent sensitization rather than direct cross-reactivity. Testing the full Lamiaceae herb series establishes each sensitization independently.
Children can develop rosemary contact allergy, though it is less common than in adults due to lower cumulative exposure. Cases in children typically arise from repeated application of rosemary-containing cosmetics (natural shampoos, baby lotions) rather than culinary herb handling. Parents of children with persistent scalp eczema or facial dermatitis should review all personal care product ingredient lists for rosemary extract, which is increasingly prevalent in 'natural' and 'organic' product lines marketed for families. If a dermatologist's evaluation is warranted, patch testing can be performed in children using age-appropriate protocols. A pediatric dermatologist or allergist experienced in contact allergy is the appropriate specialist.
The delayed onset of rosemary contact dermatitis β symptoms appearing 24β96 hours after exposure rather than immediately β is a direct consequence of the Type IV (T-cell-mediated) immune mechanism.
