Does Molina Healthcare Cover Wegovy?

The verdict

Conditional on most plans

Molina Healthcare, a Medicaid-first plan, covers Wegovy for weight loss only in the minority of states whose Medicaid program includes GLP-1 obesity coverage — 13 states as of January 2026. In most Molina states Wegovy for obesity is excluded, though a non-weight-loss FDA indication such as the cardiovascular indication may be covered.

  • As of January 2026, only 13 state Medicaid programs cover GLP-1s for obesity — down from 16 in October 2025 after California, New Hampshire, Pennsylvania, and South Carolina dropped coverage.

  • Wegovy is FDA-approved for chronic weight management (BMI 30+, or 27+ with a weight-related comorbidity) and cardiovascular risk reduction — it is not a diabetes drug and cannot be covered through a diabetes benefit.

  • Molina Healthcare operates Medicaid managed-care plans across multiple states; Wegovy weight-loss coverage is entirely determined by the individual state Medicaid PDL, not by Molina's own formulary decisions.

Medicare vs. commercial coverage

Whether Molina Healthcare covers Wegovy depends heavily on which kind of plan you hold. They follow completely different rules — here's each one, side by side.

Medicare

Part D & Advantage plans

Conditional

Molina operates Medicare Advantage-PD (MA-PD) plans in select markets. Under Medicare Part D, weight-loss coverage for Wegovy is excluded by federal statute — this applies to all Medicare plans including Molina's. The cardiovascular risk reduction indication is coverable under Part D with prior authorization for members with established CVD who are overweight or obese. The Medicare GLP-1 Bridge ($50 per month, launching July 1, 2026 through December 31, 2027) provides weight-loss access outside Part D for eligible members meeting BMI criteria; members already on a Part D-covered GLP-1 are ineligible. Medicare beneficiaries cannot use the NovoCare savings card.

Commercial / Employer

Group & marketplace plans

Conditional

Molina's primary line of business is Medicaid managed care. Medicaid weight-loss Wegovy coverage is entirely state-dependent: as of January 2026, only 13 state Medicaid programs cover GLP-1s for obesity — the count fell from 16 in October 2025 after California, New Hampshire, Pennsylvania, and South Carolina dropped coverage; North Carolina dropped in October 2025 and reinstated in December 2025. Where covered (for example, Michigan Medicaid lists Wegovy as a preferred anti-obesity medication with prior authorization, quantity limits, and eligibility for members age 12 and older), nominal Medicaid cost-share applies. In states where obesity coverage is excluded, all states retain coverage of non-weight-loss FDA indications such as the cardiovascular indication. Molina has a limited commercial and ACA book; weight-loss coverage there is employer-dependent and generally excluded on ACA individual plans. Verify on your state-specific Molina PDL.

The 5 boxes you must tick to get approved

Commercial plans that will consider Wegovy require prior authorization. Check off what applies to you to see how close you are — this is a guide, not a guarantee of approval.

Your cost, three ways

What Wegovy actually costs you swings enormously depending on how it's covered. Here's the realistic range.

No coverage

Paying cash

$1,349 per month (Novo Nordisk published list price for a 28-day supply)

List price with no coverage — before any manufacturer savings card.

PA approved

With coverage

Medicaid nominal cost-share where covered (typically $0 to $4 per prescription depending on state). NovoCare self-pay program approximately $349 per month for standard injection doses where no Medicaid coverage exists.

Typical monthly copay once prior authorization is approved.

Lowest cost

Bridge program

NovoCare Wegovy Savings Offer (as little as $25 per month for commercially insured members) is not available to Medicaid beneficiaries. From January 1, 2027, Novo Nordisk has announced a list-price reduction to approximately $675 per month.

Reduced copay through the assistance program, where eligible.

All figures are estimates. Actual Medicaid cost-share depends on your state's Molina plan rules. Verify on the state-specific Molina formulary or call Molina member services.

Check YOUR exact coverage in 3 steps

General answers only get you so far. Here's how to confirm your own plan's decision in a few minutes.

  1. 1

Find your state's Molina Medicaid PDL

Visit the Molina Healthcare website, select your state, and access the Medicaid preferred drug list or formulary. Search for Wegovy (semaglutide 2.4 mg) under anti-obesity medications. If it is not listed for obesity, your state does not currently cover it through Medicaid.

  2. 2

Ask your prescriber to confirm prior-authorization criteria

If your state Medicaid covers Wegovy for obesity, your prescriber will need to document your BMI, any qualifying comorbidities, and in some states, prior participation in a lifestyle program. Your provider can contact Molina's prior-authorization line for state-specific criteria.

  3. 3

Confirm whether a non-weight-loss indication applies

If your state excludes Wegovy for obesity but you have established cardiovascular disease and are overweight or obese, ask your provider whether the cardiovascular risk reduction indication qualifies you for coverage. All Molina states cover applicable non-weight-loss FDA indications.

Options if Molina Healthcare won't cover Wegovy

A denial isn't the end of the road. These are the paths people commonly explore next — discuss any of them with your prescriber.

State Medicaid GLP-1 obesity coverage (13 states, January 2026)

Michigan, New York, Texas, and other states on the KFF 13-state list cover Wegovy for obesity with PA and quantity limits through Molina Medicaid. Check the current KFF Medicaid GLP-1 tracker and your state PDL — the list changes as states add or drop coverage.

Wegovy cardiovascular indication

If you have established cardiovascular disease and are overweight or obese, Wegovy's CV indication may be covered even in states where obesity coverage is excluded. Ask your provider about this pathway.

NovoCare self-pay

Novo Nordisk's NovoCare program offers approximately $349 per month for standard injection doses with no insurance. Introductory pricing of approximately $199 per month for starting doses (0.25/0.5 mg) is available for first two fills through December 31, 2026. Not available to Medicaid beneficiaries at government-program pricing.

Compounded semaglutide

Compounded semaglutide, available through licensed telehealth platforms, does not require Medicaid prior authorization. Starting from $49 per month. Compounded semaglutide is not the same medication as Wegovy and has not been approved by the FDA.

These options are listed for information only — your own clinician decides what is appropriate for you.
