Criteria for Naltrexone/Bupropion (Contrave) Coverage
Applicants must have a body mass index (BMI) of 30 kg/m² or greater, or a BMI of 27 kg/m² or greater with at least one weight-related comorbidity such as hypertension, type 2 diabetes mellitus, or dyslipidemia.
The member must be at least 18 years of age.
The member must have demonstrated unsuccessful previous attempts at weight loss through diet, exercise, or behavioral intervention.
Therapy must be prescribed by or in consultation with a healthcare provider who specializes in obesity management.
The member should agree to adhere to a sustained weight management program involving dietary changes, physical activity, and behavioral therapy alongside the medication.
An initial evaluation period of 16 weeks is covered to assess effectiveness and tolerability. Continued coverage is dependent on achieving significant weight loss, as defined by a specific percentage reduction in baseline weight, as per the medical guidelines.
The prescription of Naltrexone/Bupropion should be free from contraindications such as chronic opioid use or uncontrolled hypertension.
The member must agree to regular follow-up appointments to monitor progress, side effects, and any necessary adjustments to the dosage.
