- Ankylosing Spondylitis
- Psoriatic Arthritis
- Non-radiographic Axial Spondyloarthritis

For Plaque Psoriasis:
- The patient must have moderate to severe plaque psoriasis, indicated by lesions covering at least 10% of their body surface area or severe localized psoriasis (e.g., scalp, palms, or soles).
- The patient must have had an inadequate response to at least one prior systemic therapy, such as methotrexate, cyclosporine, or phototherapy.

For Psoriatic Arthritis:
- The patient must have active psoriatic arthritis.
- They must not have responded adequately to at least one Disease-Modifying Antirheumatic Drug (DMARD), such as methotrexate, leflunomide, or sulfasalazine.

For Ankylosing Spondylitis:
- The patient must have active ankylosing spondylitis.
- An inadequate response to at least two or more non-steroidal anti-inflammatory drugs (NSAIDs) is required.

For Non-radiographic Axial Spondyloarthritis:
- The patient must have objective signs of inflammation as shown by elevated C-reactive protein (CRP) and/or MRI, and must have not responded adequately to NSAIDs.

All patients must meet age requirements as per the FDA indication for Cosentyx.

Documentation must be provided demonstrating the failed prior treatments and current diagnosis supporting the need for Cosentyx therapy.
