Small Fel d 1 particles below 4.7 micrometers in diameter penetrate to the small airways and remain airborne indoors for up to two weeks, creating continuous low-level allergen challenge for sensitized individuals. Individuals with both cat sensitization and asthma face the highest risk of severe attacks, particularly during high-exposure events such as entering a heavily cat-occupied home. HEPA air purifiers reduce airborne Fel d 1 levels by 75-90% in the rooms where they are placed, as documented by Green et al. (JACI 1999). However, the clinical benefit is inconsistent in practice. Wood et al. (AJRCCM 1998) found that despite achieving significant Fel d 1 reduction with HEPA filtration, there was no measurable improvement in symptoms, peak flow measurements, or medication use as long as a cat remained in the home. This paradox likely reflects the persistent settled allergen reservoir in carpets and upholstered furniture, which continuously re-aerosolizes regardless of airborne reduction. HEPA filtration is therefore most effective as part of a comprehensive environmental management strategy — combined with hard flooring, allergen-proof bedding, and keeping cats out of the bedroom — rather than as a standalone intervention.
