Immunocompromised patients receive a 5th vaccine dose on day 28. Previously vaccinated patients need only 2 vaccine doses on days 0 and 3 and do not receive HRIG. The schedule is identical for Imovax and RabAvert.
You can refuse any medical treatment, but the clinical consequence of refusing rabies PEP after a credible exposure is potentially fatal: clinical rabies is essentially 100 percent fatal once symptoms develop, and only a handful of survivors have been documented in modern medicine. Every authoritative guideline โ CDC, AAAAI, WHO โ explicitly states that PEP should not be withheld for allergy concerns because the math overwhelmingly favors vaccination under medical supervision over refusal. If you have a documented vaccine component allergy, discuss product selection (Imovax vs RabAvert) and supervision level with the emergency clinician, but do not delay or refuse PEP.
A serum sickness-like reaction is a delayed Type III immune-complex phenomenon that appears 1 to 3 weeks after a rabies vaccine dose and presents with low-grade fever, urticarial or maculopapular rash, and polyarthralgia (pain in multiple joints). Complement levels are typically normal, distinguishing it from classic serum sickness. It is self-limited within 1 to 2 weeks and responds to supportive care with acetaminophen, oral antihistamines, and occasionally a short course of oral corticosteroids in more severe cases. It is not an IgE-mediated anaphylaxis, does not require epinephrine, and does not contraindicate future rabies vaccination.
Rabies PEP should begin as soon as possible after a credible exposure. There is no hard cutoff time after which PEP becomes futile if the patient is still asymptomatic โ there are reports of successful PEP even days to weeks after exposure โ but earlier is always better because the incubation period can be as short as a few weeks. In practice, PEP should start within 24 hours of exposure whenever possible. The 4-dose schedule (days 0, 3, 7, and 14) is then completed on time. Do not delay seeking care to shop for products, to complete allergy testing, or to confirm whether the animal was actually rabid.
Not routinely. Most people who have completed pre-exposure vaccination do not need booster doses unless they have ongoing high-risk occupational exposure (laboratory workers handling live rabies virus, certain wildlife biologists) or develop a credible new exposure. For standard travelers and occasional wildlife contact, pre-exposure vaccination provides durable protection, and any new credible exposure is managed with 2 additional vaccine doses on days 0 and 3 without HRIG. High-risk occupational workers receive periodic serology to confirm protective titers and booster doses when titers decline.
