The manufacturing process has not been replaced with a cell-culture alternative, so the residual egg protein content is an unavoidable feature of the product. Combined with the highest reported anaphylaxis rate of any routine vaccine (4.2 to 8.0 cases per million doses), this makes egg allergy a formal contraindication that the ACIP has not retired, unlike the 2023 influenza vaccine egg precaution removal.
Desensitization for yellow fever vaccine is a one-time graded-dose administration procedure performed by an allergist, typically over 3 to 5 hours at an academic drug allergy center. It begins with skin prick testing using undiluted YF-VAX, then intradermal testing at 1:100 and 1:10 dilutions if the prick is negative. Anyone with a documented reaction to a prior flu shot or MMR that raises concern for IgE-mediated hypersensitivity should be evaluated by an allergist before yellow fever vaccination, regardless of how the flu reaction was ultimately classified.

YEL-AVD stands for yellow fever vaccine-associated viscerotropic disease β a rare severe multi-organ failure presenting 2 to 8 days after vaccination with fever, hypotension, and a clinical picture resembling wild-type yellow fever. YEL-AND stands for yellow fever vaccine-associated neurologic disease β encephalitis, meningoencephalitis, or a Guillain-BarrΓ©-like illness 4 to 14 days after vaccination. Both are rare (approximately 0.3 to 0.4 cases per 100,000 doses), more common in adults over 60 and in patients with thymus disease, and both are thought to involve host-specific factors affecting vaccine virus control rather than hypersensitivity. They are not allergic reactions and are not treated with epinephrine.
No. In 2016, the World Health Organization formally determined that a single dose of yellow fever vaccine provides lifelong protection, and the International Health Regulations were updated accordingly. Most endemic-region countries now accept the original ICVP (yellow card) as valid for the traveler's lifetime without a booster requirement.
